Operational risk, delivery risk, reputational risk and the cost of change. The four horseman of the Ops apocalypse. As organisations grow, they inevitably begin to encounter these risks, and yet a surprising number of businesses do not recognise or mitigate them. The smooth running and performance of the production environment are treated as a problem for only one team to solve, disregarding both the importance of these problems, and the relevant skills that others in the organisation may have. The end result is botched releases, outages, and lost customers.

Instead, forming cross-functional teams and learning to recognise (and solve) common production antipatterns can lead to markedly safer release, and fewer outages.
