Overview

Failure is inevitable. Success is unpredictable. Both failure and success are also tremendous learning opportunities. A postmortem, conducted in an skillful way, is one of the best ways to learn from both failures and successes. Postmortems can help us identify and address areas of fragility within systems and organizations, and make both more resilient.

What you will learn

In this highly-rated, practical, hands-on session, you’ll learn the theory and, most important, get to practice conducting awesome postmortems, including:

  • How to set the context, and go beyond blame
  • What data to collect before the postmortem and how to present it in the most useful way
  • How to facilitate charged conversations during high-stakes postmortems
  • Finding the real root cause of any failure or success (yes, there is such a thing!)
  • Learning in complex, adaptive systems
  • The new view of “human error”
  • How to recognize and mitigate the effects of cognitive biases
  • Writing up postmortem results (for internal and external audiences)
  • Harnessing prospective hindsight to identify risks and reduce the possibility of future failures

Dave Zwieback has been working with large-scale mission-critical infrastructure and teams for almost two decades. Dave is the VP of engineering at Next Big Sound (acquired by Pandora Media, Inc.) and CTO of Lotus Outreach. He has previously worked with the adaptive learning startup Knewton, the quantitative investment management firm D.E. Shaw & Co., and the financial services behemoth Morgan Stanley. He also ran an infrastructure architecture consultancy for seven years. Dave is the author of Beyond Blame: Learning from Failure and Success from O’Reilly Media. He blogs at Mindweather.com.

Coaches Training Institute

Leonid Frolov, CPCC, is a highly-sought after coach, trainer and leadership facilitator who provides individual and group coaching to a variety of private individuals, groups and organizations.

Leonid holds a Certified Professional Co-Active Coach (CPCC) certification through The Coaches Training Institute (CTI), and is a graduate of their Leadership program. In addition to his private practice, Leonid serves as a member of the certification faculty at CTI.

In partnership with COPE, Leonid is a co-designer and facilitator of a program that provides coaching for employees of the World Bank and their spouses. He is also a co-founder of the Crossing Borders with Confidence partnership, which launched in September of 2014.

Leonid is a co-designer and facilitator in a series of workshops “Finding Your Powerful Voice” throughout the world.

Leonid grew up in Russia where he received a B.S. in geology and later a Masters in Linguistics from St. Petersburg State University.
